CARF Accreditation Ready Joint Commission (JCAHO) Ready

CARF & Joint Commission Compliance Support

myKokoro EHR is built to support CARF-accredited and Joint Commission-accredited behavioral health practices. Every documentation requirement, supervision record, and audit trail is built in — not bolted on.

CARF 2026 AI Governance Requirement — Effective July 1, 2026

CARF's ASPIRE to Excellence framework now requires accredited organizations that use AI tools in service delivery to have a written AI governance policy. If your practice uses myKokoro's AI-assisted features, you need a policy on file before your next survey. myKokoro provides everything you need to meet this requirement — including a ready-to-use policy template below.

How myKokoro Satisfies CARF Requirements

Clinical Documentation

Progress notes, treatment plans, discharge summaries, and assessments — all with locked, timestamped records. Every document has an author, a date, and a version history.

Supervision Documentation

Pre-licensure supervision hours, co-signature workflows, individual and group supervision logs, and staff evaluations — all stored in the system and exportable for survey documentation.

Outcome Measurement

15 validated screening instruments with auto-scoring and score trend charts. Measurement-based care built into the clinical workflow — not a separate system.

Compliance Tracking

Staff training records, CEU tracking, license management with expiration alerts, and practice recredentialing — all in one place.

Audit Trail

Every action in the system is logged with user, timestamp, and device. The built-in audit viewer lets you show a surveyor exactly who accessed what and when.

Chart Deficiency Reports

Run a self-audit before a surveyor arrives. The Chart Deficiency / Compliance Report shows every active client with missing documentation — treatment plans, overdue assessments, unsigned notes, and more.

What Your Practice Needs to Do

1

Write your AI Governance Policy

Use the template below. Fill in the bracketed fields with your practice name, AI coordinator, and review date. This document satisfies CARF's written policy requirement under the ASPIRE to Excellence framework.

2

Designate an AI Coordinator

CARF requires that someone in your organization is responsible for overseeing AI use. This can be the practice owner, clinical director, or any qualified staff member. Document their name and role in your policy.

3

Review and Update Annually

Your AI governance policy must be reviewed at least annually. Set a calendar reminder. When myKokoro adds new AI features, update your policy to reflect them.

AI Governance Policy Template

ARTIFICIAL INTELLIGENCE GOVERNANCE POLICY

[Practice Name] — Effective Date: [Date] — Review Date: [Annual Review Date]

1. PURPOSE

This policy establishes the governance framework for the use of artificial intelligence (AI) tools within [Practice Name] in accordance with CARF International's ASPIRE to Excellence framework (effective July 1, 2026) and applicable federal and state regulations governing behavioral health services.

2. SCOPE

This policy applies to all staff, contractors, supervisees, and interns who use AI-assisted features within the myKokoro EHR platform in the course of providing services at [Practice Name].

3. AI TOOLS IN USE

The following AI-assisted features are currently in use or authorized for use within myKokoro EHR:

  • AI note drafting (currently beta testing; when enabled by practice): Converts clinician session dictation into structured clinical progress notes. All generated notes are reviewed, edited as needed, and signed by the licensed clinician of record before being locked in the client's chart.
  • Automated Assessment Scoring: Validated screening instruments (PHQ-9, GAD-7, C-SSRS, and others) are scored automatically based on client or clinician-entered responses. Scores are interpreted using published clinical thresholds.

4. HUMAN OVERSIGHT

No AI-generated content is used in a client's clinical record without review and approval by a qualified licensed clinician. AI tools assist documentation — they do not replace clinical judgment. All treatment decisions, diagnoses, and clinical recommendations are made by licensed staff.

5. DATA PRIVACY AND SECURITY

All AI processing within myKokoro EHR occurs within a HIPAA-compliant infrastructure. Protected health information (PHI) is not transmitted to third-party AI providers without a Business Associate Agreement (BAA) in place. myKokoro EHR maintains a BAA with all applicable AI service providers.

6. STAFF TRAINING

All staff authorized to use AI-assisted features will receive orientation on the appropriate use, limitations, and oversight requirements of those features prior to use. Training completion is documented in the staff record within myKokoro EHR.

7. AI COORDINATOR

The designated AI Coordinator for [Practice Name] is [Name, Title]. The AI Coordinator is responsible for overseeing compliance with this policy, reviewing AI tool updates, and ensuring annual policy review.

8. POLICY REVIEW

This policy will be reviewed annually, or sooner if new AI tools are adopted or if regulatory guidance changes. The next scheduled review date is [Annual Review Date].
